i had already filed my 2008 tax return and then got a 1099-r for a roth ira distribution that i had forgot about in 2008. the cost basis was more than the distribution itself so i had a loss on the roth. there is no reportable income but do i need to do a 1040x? when i do a 1040x the adjusted gross income is the same and so forth so what do i need to do? thanks

A good set of notes throughout the year may help avoid errors like that.

I suspect the IRS may send you a note later this year or next.

File form 8606 with part III completed. And add a note that explains what is going on.
Form 8606 can be a stand alone form.
